Instead of Azazel and ? basically having free devil deals (and Lilith but shoot Lilith would be nerfed to high hell with this) you'd have to play well to actually earn them, so that Azazel can have a drawback to being able to easily clear the early floors ("yeah, you're strong, but if you want to be stronger you'll have to actually master him") and with ?, well, he was the original challenge character, so that would make some sense for him. That way, you wouldn't be punished for taking "base health" damage by losing items that would make it easier to not take damage in the first place, and players would have to make a choice: get soul/black hearts for an "easy way around the health cap," or risk going around with lower health but a better chance at getting devil and angel items. This would be pretty mediocre if Beth didn't come bundled with one of the niftiest items in the game: Book of Virtues!
